Getting certified in Arizona is simple with Green Bridge. Whether you're a first-time patient or renewing, we’ll walk you through it.

✅ New Patient Certification – Arizona

Step 1: Get Certified by a Doctor
Our doctor will conduct a quick telemedicine appointment to confirm you meet the qualifying conditions. Once certified, we’ll provide you with the completed Physician Certification Form.

Step 2: Apply Through the Arizona DHS Portal
You (or your caregiver) must submit your application online. You can do that HERE

What You’ll Need to Upload:

  • Completed Physician Certification Form

  • Arizona Driver’s License or State ID (valid and current)
    – If you don’t have an AZ ID, you’ll need two documents showing AZ residency

  • Digital passport-style photo

  • Completed Patient Attestation Form (this is part of the application process)

  • $150 state fee ($75 if on SNAP)

📌 SNAP recipients must upload a current SNAP benefit card or letter.

Step 3: Wait for Approval
After submitting everything, the Arizona Department of Health Services typically processes applications within 5 business days. Once approved, your card will be sent via email as a digital file—no physical card is mailed.

🔄 Renewal Certification – Arizona

Step 1: Renew Your Certification with a Doctor
Our doctors will re-certify your condition and issue a new Physician Certification Form.

Step 2: Submit Renewal Through the State Portal
Use the same portal:

You'll need to re-upload:

  • Updated Physician Certification Form

  • Valid Arizona ID or residency documents

  • New passport-style photo

  • SNAP card (if applying for reduced fee)

  • $150 state fee (or $75 with SNAP)
