Here's what you need to know before you begin: ⏰ Appointment Windows (Not Exact Times) When scheduling, you’ll select a Morning, Afternoon, or Evening time window. Please note: 🕑 The time shown is not your exact appointment time. Our doctors call patients throughout the chosen window — usually within a few hours. Calls may come from a private or spam-labeled number, so keep your phone nearby and turned on. 📋 Fill Out the Intake Form Carefully Once you schedule, you'll be asked to complete an intake form. This step is required before your certification can be issued. ✅ Double check your: Last Name and Date of Birth – this must exactly match the information in the state system Phone Number – the doctor will call this number Email Address – this is how we send confirmation and state instructions ❌ Incorrect information = delays in your certification. 🔎 How Doctors Locate Your Record Doctors verify your identity using your last name and date of birth — that's how they locate you in the system. If your info doesn't match what the state has on file, we won’t be able to process your certification until it’s corrected.
