Navigate to your state’s patient login page.

Once you are on the login page, click the link for “Forgot Username or Password”

Follow the instructions. Typically enter your email address and request a reminder of your username or a link to enter a new password. Passwords need to be 8 characters and include both upper and lower case letters, a number and a symbol such as ? or !.

If you no longer have access to the email address you used to register your account, you can either reach out to your state to have that updated or send us a help request and we can assist you with that.
